SELF-ASSESSMENT QUIZ
MALARIA: KEY FACTS
300 - 500 million cases each year
700,000 - 2.7 million die each year
600 cases in US in 1914
Malaria not endemic in US (1940s)
World population 41% endemic
Africa, Asia, Middle East, Central & South America, Hispaniola, Oceania
Mosquito, blood/blood products, congenital, shared needles
Case Study 1
Jul '06 47 year-old male traveler returned from Sub-Saharan Africa, low grade fever, malaise, some diarrhea, had taken incorrect prophylaxis
Testing: Thick and thin blood film prepared; read that evening; both blood films reported as negative; blood films examined by two differerent individuals the following morning
Final Report: Patient diagnosed with Plasmodium species; "Unable to rule out Plasmodium falciparum"
Case Study 2
May '03 21 year-old male returned from Iraq and Afghanistan.
June '03 Cyclic fevers, chills, rigors, severe malaise; diagnosis with malaria (Plasmodium vivax), patient remained in-house
Appropriate therapy - BUT patient intubated, respiratory failure
Sept '03 Patient discharged after very difficult course
Pleuritis, chest pain, fevers (103), 0.3% parasitemia, appropriate therapy with chloroquine and primaquine
Oct '03 Smear negative
Nov '03 Smear negative
Dec '03 Fevers, rigors, 0.8% parasitemia, high dose of primaquine - patient remains ok
Primaquine tolerant/resistant P. vivax
Case Study 3
A 43 year-old male from Maine returned from a trip to Africa. On his return, he was in an auto accident and required a blood transfusion. After several weeks post-transfusion he had fever, headache and general malaise. Since he lived in Maine and received a blood transfusion babesiosis was suspected. However, symptoms were caused by Plasmodium falciparum (Africa). He had taken prophylaxis but did not continue to take it after coming home. Because of very low parasitemia it was undetectable by automation (hematology instrument). By the time P. falciparum was confirmed he was very ill and almost died.
Fifth Plasmodium species: Plasmodium knowlesi
24 hr (simian malaria); rapid cycle
Tends to infect any RBC; heavy infection
All sizes of RBCs; Duffy factor like P. vivax
Delicate rings; may have two dots of chromatin ring; applique or accole forms; may have two rings/RBC
Band form trophozoites
Gametocytes round, tend to fill the cell
